Where are claims mailed?
Claims should be mailed or filed electronically by your provider to the address on the back of your ID card.

Who should my provider or I call if I am going to be admitted to the hospital?
To pre-certify a hospital admission. Contact the Pre-certification phone number listed on your ID card.

Can PHA provider nation-wide access to healthcare?
Yes. PHA offers access to medical care to employees and beneficiaries in all states plus District of Columbia.

Is PHA and Insurance Company?
PHA is not a health plan, and we do not underwrite risk. A major advantage of PHA is our “portability”. Clients can use the PHA managed care system with their existing indemnity plan and TPA or insurance carrier. PHA is willing to work with any carrier/claims administrator.

How many providers are part of PHA?
More than 400,000 physician, hospital and other health care providers are PHA participating providers nationwide.

How current are PHA’s online directories?
Our on-line provider search capability is updated daily. You can also verify a provider’s participation by calling our toll-free customer service line.

How does PHA channel patient to its network?
PHA encourages strong benefit plans that direct or channel participants to PHA network providers within state-specific guidelines for directing care. Increasing network penetration means more health care cost savings for payors and patients.
